I'd backpack around the world, dig wells for people, raise awareness about various issues.

i would travel and act like a retired queen with a lot of cash to spend on myself.

I would go volunteer with an organization on a permanent basis and use my knowledge in medicines to help people. I love doing it and I do it once a year. But, I have to work to provide for myself otherwise I cannot take time off to do something like volunteer....

If, by saying 'didn't have to work', you mean that there is still plenty of money, I would travel a LOT. I would also be a perpetual student. I love to learn about lots of things, just about anything. I would probably still work, or volunteer at least, as a paramedic. working as a medic overseas might be fun :)
