Depends on the particular taste of the terrorist. Some like to give a warning, some don't

Yes. a bomb would definitely be a good reason, but I think certain mechanical problems or other non terrorist reasons could cause this to happen, although airplane checks and safety are continually being upgraded and improved.

In-flight explosions are extremely rare, and a fair number of those that have occurred have been the result of deliberate sabotage (such as terrorist activity). When it has not been sabotage, it has usually been detonation of fuel-air mixtures inside the fuel tanks, as in the case of TWA 800.

Today methods such as pumping nitrogen-rich air into the fuel tanks are used to diminish the (already unlikely) probability of a mid-air explosion.
