It is called the Wright Amendment. Certain airlines are contracted to fly in and out of certain airports. Like Southwest's main hub is in Dallas, but they aren't allowed to fly into Dallas/Fort Worth International. You can do a google search on Wright Amendment. =)

There are a variety of reasons.

In most cases it all comes down to whether or not the airline can make money out of the route. That is, if there is anough demand/passengers or cargo to make it viable.

Other reasons can be more varied; Whether they can get permission/slots/landing rights at certain airports; Wheher or not they have the appropriate aircraft to use the airport or whether the airport can handle their aircraft and needs (some small airports cannot handle larer planes and don't have the ground facilities or runway length or strength to accommodate it); And it can sometimes be that it is simply not technically feasible to fly directly from one port to another.

Many reasons. A large airline like United or American would be foolish to fly into a tiny airport in the middle of Montana. Their clientele simply wouldn't pay for the cost.

Most airlines operate on a 'hub and spoke' system, which is the most effective way to move people between multiple points in the least time. Every airline has different airports they use as main hubs, such as Delta uses Atlanta.

The airlines try to maximize profits by keeping their seats full. Small airports or cities may not have enough passengers to make it worth while to go there so people are moved to larger cities and consolidated to fill larger airplane and fly longer distances. With the high cost of fuel today, it is even more important to keep the seats full.
