in public, it's "OUCH!!!!!"

at home however, it's usually "arayyyyyyy ko poo...ang sakit sakit naman" (it really hurts!!!) and then i bawl like a baby!!!!!!

you can say either "ouch" or "aray" or the taglish "arouch". or if you want, you can say expletitives AT YOUR OWN RISK [not recommended] or use terms or synonyms of the phrase "that hurts" (in any language that you're fluent with).
