Not so well as many Malaysians like to think.
Generally, those of Indian ethnicity speak BY FAR the best standard of English amongst Malaysians.

Most Malaysians speak English. There are two types, one is the Standard Malaysia English (ME) and the other one is Manglish with heavy local slangs, just like Singapore's Singlish. It is common that most people in Kuala Lumpur juggle with 2 to 3 languages daily.

Put it this way- English tourists have absolutely no problem communicating here.

Malaysian is using Malayian English and Manglish, with almost all words ending with "lah".
"Yes laa!"
"No lah"

wkc: only some is right.

But other than that, we understand English very well. Tourists never had any problem in communicating English with the locals. It's among the country chosen by tourist to traveled without any language barrier.

Edit:
Agreed with Neilisa and edifier. I do not converse using Brits English, because its sounds too "imperious". No offense please. Just a thought.

I'd say very well compared to our fellow Asian counterparts, lol=P
The fluency rate in this country is pretty high. So expatriates generally have nothing to worry 'bout.
Malaysians speak the Queen's English (UK) btw, although the younger generation tend to practise the American English more due to popular culture.
