Health card yes

S.I.N No

Here is all the info you will need too get your Manitoba Health Card. The address and phone number is on the bottom of the page

Health card is provincial, so yes.

SIN card is federal so no.

Z
Health care is a provincial matter and so, yes, you will have to contact Manitoba Health. Here is their link http://www.gov.mb.ca/health/.
Your SIN card, however, is one issued by the federal government and so you will not have to apply for one.

Health card is provincial so you have to apply for that, SIN card is federal so it can travel with you.

Just a new health card. Each province has jurisdiction over its own health care system.

Steve
Health yes, SIN no.

Health cards are issued by your province of residence. If you become a resident of Manitoba then you should apply for a Manitoba health card. On the other hand, Social Insurance Numbers are issued by the federal government and are valid across the entire country.

Melanie B
Yes you need to apply for a new health card. You are still covered under OHIP for a period of time.

No you don't need a new SIN card.
