SIN card is federal so no.

Health care is a provincial matter and so, yes, you will have to contact Manitoba Health. Here is their link http://www.gov.mb.ca/health/.
Your SIN card, however, is one issued by the federal government and so you will not have to apply for one.

Health yes, SIN no.
Health cards are issued by your province of residence. If you become a resident of Manitoba then you should apply for a Manitoba health card. On the other hand, Social Insurance Numbers are issued by the federal government and are valid across the entire country.

Yes you need to apply for a new health card. You are still covered under OHIP for a period of time.
