no you must have state issued photo ID and birth cert. I go to canada frequently live in toledo oh and forgot birth cert once and was stuck there for 6 hours.

You only need a passport to cross the boarder if you are flying. You do not need a passport to cross the boarder in your car. You do need government issued photo id and a birth certificate. Usually a heath card (even if it has a photo on it) is not accepted as a form of id simply because you should not give anyone your health card because it could have sensitive information on it. You might get lucky and they will accept a health card, but you are taking a chance. A birth certificate is a must.

as a canada border security agent i can answer this
i talk to a us border officer this morning in detroit and the answer is this
you need 2 pieces of id one must be a driver licence or photo id issues by a government agencies no school or university id are accepted
second id must be a birth certificate to confirm where you were born
you friend can contact the nearest us consulate or embassie if living on ontario it ottawa and toronto ( he r she must go there) and recieved a autorisation to cross the border from the us gov i assume your are either from quebec or ontario since they are the only two province that have photos id on their health care so that is the way if not she may not be allowed t cross over in the usa and if caught she or he can be denied access again in the usa for 5 years

I am pretty sure anything formal and verifiable with a picture is sufficient. student ID? Occupational ID .. something like that .. but it has to be laminated..
I went into canada 2 months ago ( I am 31) security didnt even ask for any ID from the passengers. Going both directions.
I don't have a definite answer for what the standard is. Just basic questions .. Where ya going? What are you doing? Are you crossing the border with anything. No search. No drama.
But, I had my drivers lisc.

I am from Toronto and just got back from a road trip to New York City. I got through with no probs using my health card. One friend had a drivers license and the other a birth certificate with pic id. Your friend should have no probs.

ONE WARNING: coming back into Toronto, you'll prob want to stop off at duty free. They have an amazing deal for two 40oz. bottles of alcohol for $25. Little did we know you were only allowed to bring one bottle each across. They made us either pay $30 for each additional bottle or go back across the border to return them.
