Puerto Rico is NOT a state, it is a Commonwealth and territory of the USA. Its residents are U.S. citizens. You don't need a passport to go there but I would advise having one for I.D. purposes only! I never travel without one anymore.

I actually saw a guy claiming to be cuban with a permanent residency in the U.S. stopped by what seems to be INS agents standing at the gate of the airport asking the question:
"Where were you born?"
Eventhough I am an american citizen I was not born in the US. When I answered the question with, I was born in country XYZ (not the USA) I was immediately asked to show identification - thankfully I never travel without my passport and I went right through. Now for our cuban friend he was stopped and the agent was very polite telling him that only US Citizens do not need a passport.
I am not sure what would have they done to me if I did not have my US passport handy. I can only guess they would have asked me questions like, where did you become a natural citizen, what city, which INS office, etc? etc? Or things like, what Military base where you born while your folks were serving abroad?
To make a long story short and to avoid unconfortable questions I would just bring my passport just in case.

it's not a US state, but a commonwealth of the U.S.
Its citizens are American citizens and can travel to and from the mainland without a passport. Mainland Americans can do the same.

No, if using a domestic flight to get here. If traveling from Spain to PR then yes.
Until Homeland Security changes the rules for domestic flights, you will only need a state identification (driving liscense) to travel here from the states.
