Thank you for your question.
It need not. However, you would need a marriage certificate or information verifying a change of name. You will need a US passport and possibly a visa.
To obtain a passport for the first time, you need to go in person to one of 7,000 passport acceptance facilities located throughout the United States with two photographs of yourself, proof of U.S. citizenship (such as a birth certificate), and a valid form of photo identification such as a driver’s license.
I have enclosed information on applying for your information.

Those FEW places that still allow birth certificate and driver's license/non driver's id as proof of citizenship understand that most women particularly have a different last name now than is on their birth certificate. I traveled for over 20 years to the Caribbean on my birth certificate and driver's license with no problem. however, it would be best to go ahead and obtain a passport as passports will be required to ALL foreign countries before end of the year. |

As someone who travels alot and just got my passport. I can tell you this. As of Jan 1, 2007, you will need a passport to travel outside the US. The total cost is $112 and that includes having your photos taken. Part of the application will go into previous names. I am pretty sure the government realizes that the last name on a Birth Certificate my not match your current last name (divorce, marriage are the 2 biggies). Just follow the links given by the poster above. Just remember it takes 6-8 weeks to process. |

If your last name is different then your birth certificate, bring your marriage license, or any official documentation to support your current name. You will need to get a passport 1-1-2007.
You will need a drivers license as well. |

I don't know about traveling.. but for most things in the US, you need to use the last name that is on your Social Security card. My son has 1 last name on his BC and a different on his SS card. For taxes and everything using his social I have to use the last name that matches it. If not it gets kicked out for incorrect information. I would ask a travel agent or an attorney |

If you are traveling the the Caribbean you will need an original birth certificate and an original marriage certificate. Your drivers license must state your new name. If you are traveling other than the Caribbean, get a passport. |
