Yes, you need a Jamaican passport. You will not be allowed on an international flight without a passport. You also need to carry your green card.
If you do not have a current Jamaican passport, you can apply for one through the website of the Consualte General of Jamaica. See site below ans click on the word " pasports" in the list on the left.

If you enter Jamaica on a foreign passport, the same restrictions as other visitors will be stamped into your passport. (This has only been in effect for a couple of years.) Now you can have this removed but it would take time to go to the immigration authorities in Kingston and who needs the hassle. It is best to get a Jamaican passport if you want an extended stay. You can return to the USA on your US passport
