In December 2006, no. You will only need birth certificate and valid photo ID. As of Jan 2007, you will need a passport.

Yes it is. Or at least it was 3 years ago when I went there. CAll thier embassy and find out for sure.

You will need a passport every time you go outside the US after January 1, 2007 so you may as well order it now and beat the rush at the end of the year.
