Is it possible to make a day trip to the Culebra Island and back to San Juan?

Do the ferries run frequently enough for you to go back to the main island, Puerto Rico, on the same day?

Yes it is possible.
The "high speed" ferry from SJ is not running. So for a ferry, you need to get to Fajardo. The cheapest way is to rent a car for the day and park it in the lot at the ferry.

Flamenco beach is great. Rent snorkel gear at the scuba shop at the ferry dock on Culebra.

Or you can fly:
From SJ, you can catch flights out of SJU or Isla Grande, but they are more expensive than from Ceiba. Again, to save $, rent a car and park it.

Well you could do it if you only stay for a few hours. The public ferry from Fajardo to Culebra takes about 1 1/2 hours to get there, so include that time. And remember weekends & holidays are crowded, you might not be able to get on the ferry. Below I included the earliest & latest departures:

To Culebra from Fajardo Mon-Fri: 9:30 am
To Fajardo from Culebra Mon-Fri: 3 pm

To Culebra from Fajardo Sat/Sun: 9 am
To Fajardo from Culebra Sat/Sun: 4:30 pm

There is supposively a Hi-Speed ferry from San Juan that also goes to Culebra. However I am not sure if it is still in service. Call to find out:

Otherwise you can take a small plane or a small private boat. But you will definitely pay more money so that is up to you. Goodluck!

No, the travels to Culebra and Vieques run from the ferry boat of Fajardo starting at 5:00am and finishing at 10:00pm

Yes is possible, better if done on midweek. You can take the 9am and back on the 4pm. The trip is like 1:15 to 1:30. Once you get to the ferry dock, buy the roundtrip tickets.

yes, my cousin does it all the time, but it would be best to take your car with you because that way you can enjoy the day and wont waste time trying to get from one place to the other. I think the ferries run 4 to 5 times per day.
