You can either start a cruise here or take a cruise from say Miami, that has a port of call in San Juan.

I went on Royal Caribbean 'Explorer of the Seas' over Christmas & Puerto Rico was the first call of port after departing from Bayonne.
I'm not sure if it's still doing that route or not, but you could look? :]
