Yes, she will need a visa or a passport
if your mom is not a U.S. citizen. I do not know her legal status, so it is impossible for me say which one she needs. Check the U.S. government's visa and passport websites to see what documents are required or accepted to fly to P.R. for non-U.S. citizens.

I do not include visa information because I am not knowledgeable about that subject.

Only U.S. citizens can go to P.R. and return with just their ID (eg.,driver's license, birth certificate, etc.). But not to worry, getting a passport is not difficult to get, nor expensive ($65.00). If she is planning to visit in a near future she should begin the process now. It does take longer to get a passport as summer approaches because many others have also applied. Get a passport application at the post office, go to Kinko's and get two pictures taken, I think 2x2s, and fill out application. You will also need a certified birth certificate, NOT an abstract. Take your packet back to the post office, pay the transaction fee, and they will tell you an approximate time when she should receive her passport. She can pay an additional fee to "rush" the process. Tell your mom I said "have fun in Puerto Rico"; she will love it!

Well,
She need a valid passport from the nation she was. If she have a valid visa to enter or stay in the US she can used (with valid passport from her nation) but need to communicate her intentions to visit PR to the US Immigration Office because if she travel and don't inform, she most be arrested when arrive at the island.
