Some speak English, some speak Spanish, but you are not likely to understand them anyway, so shout louder.
You should go to Saint Vincent and the Grenadines, which is a much nicer place, where they speak English, but you will not understand the accent either. So shout alot!

It is Puerto Rico. They speak spanish as their native language, but some speak english. We had no problem communicating while we were there. Look it up online and read the history. It is quite interesting. I have listed the site below. Hope it helps.
