10 US Dollars (and it does have to be in US no other currency) to get in
and then 20 US Dollars (once again having to be US Dollars) to get out.

from memory - if your on a package holiday your entry fee is included in your package but you need to pay again to get out.

The first poster is right - if you have booked to go on a package holiday there, the Visa Should be included.
I've booked for the Dominican Rep (and other Caribbean islands) for April and my visa for all Islands are included.

We had to pay on entering and leaving. On entering we also spent about 40 minutes (as did our whole flight) filling in immigration/visa forms.
