We just took our moms on a mediterranean cruise on the NCL gem, which was very informal. All seven nights provided a wide variety of dining choices without ever requiring anything nicer than smart casual (and many cruisers were less dressy than that). All NCL ships have the same "dress-up optional" policies. Having cruised on Holland America, Princess, Cunard, Carnival, and Celebrity, I would say NCL policy fits your requirements best. Smart Casual would not be technically "up to snuff" on several nights on all the other lines, although Princess and Carnival might let you get away with something below the requested dress code.

If you are looking for a more upscale option, I recommend Oceania Cruises, which is resolutely smart casual all cruise long.

NCL have a very good reputation and are not formal but they are not too informal like easycruise which is extremely informal and can be full of lager louts, i know this cos i used to work for them.
You should google NCL Norwegian Jewel, its a lovely ship.

I'd try Carnival. I did their Med cruise a couple years ago and it was good. Smart/casual in the dining room, all but 2 nights. There are loads of places to eat on board, so we really didn't use the dining room that much anyway.
