However, most cruise lines will deny boarding for a pregnant woman past a certain point in her pregnancy. In small part to avoid this issue, and in much larger part becuase they aren't equiped to provide neo-natal care. If something were to go wrong, the cruise line would be responsible and they aren't willing to take the risk.

That's the same answer as if they were born on an airplane in international air. They would be citizens of the country the parents were citizens of.

Since the middle of the Atlantic is "International Waters," children born there assume the citizenship of their parents, and their place of birth is registered usually as the country of registry of the ship they are on, but asterisked as being a ship in international waters. They would not qualify for citizenship of the ship's registry country usually. Same goes for airplanes.
FYI, Passenger cruise ships limit passage to pregnant women to those less than 27 weeks along, and only those with no history of complications or premature births. So it's a very rare occurence nowadays.

Most doctors won't allow a pregnant woman to travel by sea or air after a certain point so there should not be an issue like this.
If this were to happen, infant would take the birthplace of mother.
