Not at all. Most ships have swinging door balconies. Rather like bathroom stalls. In fact on my last cruise on the Carnival Glory the latch was broken for the first couple days.

Some ships have frosted balcony doors that divide them. They are mostly opaque but not really.

It also depends on what balcony room have you too. Some close to the front of the ship can actually be seen from the decks. When you walk back to the stairs you can see the balcony you are right above and see their balcony. Now a balcony with an obstructed view that was the very last one on the floor might be.

OMG, personal story time!

Ok, this one time I was on a cruise. My parents always get a room on the 10th floor with a balcony. We were standing out there looking at some flying fish and we looked down for a second and saw a women standing there with nothing on. It was so shocking. It is not a safe place for sex, that's for sure.

Basically, the balconies are separated by frosted glass or not-very-thick plastic, so you can hear everything going on next door if your out there. Most ships are "layered" and you can see down into every ones balconies from the top ones. If you decide to look down the railings and lean your head over to see the front or the back of the ship, which most people do a lot of for some reason, you can see pretty much the entire balcony of about 10 rooms down.

So, yeah, it's safe to say they are not private enough for sex at all.
