It depends on which cruise line you are asking about, but there are none that are truly all inclusive. You will have to pay for all drinks from the bar, meals in the speciality restaurants, all spa treatments, excursions, anything you buy from the shops on-board and photographs.

When you are off the boat is not included plus you have to pay for everything you buy on the boat.

Read the small print. Sometimes alcohol is not included. And I have seen it where tea, coffee, juice is included but sodas were not. It can vary from cruise line to cruise line or from hotel to hotel. So just check it out. Of course your own entertainment and food while off the ship is your responsibility.
