YES - changes to law a couple of years ago mean children cannot travel on parents passport regardless of age
My nephew had photo of him when he was 3 hours old on his passport as his parents drove to France to introduce him to the French half of the family when he was 7 days old - my brother went to Liverpool passport office to get it when he was three days old! Lady at passport office jokingly said photo could not be accepted cos he looked like he was smiling. Had same passport now and he is 4!

Yes. Everyone (even a baby) needs a valid passport to enter and leave most foreign countries. The U.S. Department of State issues passports.
Unfortunately, applying for a first-time passport has become more stringent and complicated in recent years. No first-time passport applications are accepted by mail. As a parent or legal guardian, you must apply in person on your baby's behalf and you must bring your child with you when you apply.
If your child has two parents or legal guardians, both must be present to sign the passport application. If both can't be present, you'll need a notarized Statement of Consent signed by the absent parent or guardian.
If you're the sole parent — the other one is deceased or you have sole legal authority over your child — you'll need to provide evidence of that. If you can't prove you're the sole parent or for any reason can't get your child's other parent to sign the Statement of Consent, you'll have to explain why in the Special Circumstances box on the statement. |

I was on my mums passport until I got my own when I was 12 and we never had any problems travelling abroad before. However, with the new era of heightened security, this may have changed. Ask at your post office! |

ps i am sure you can no longer put kids on a parent's passport. One reason for this is what if the parent dies? ( or is hospitalised or incarcerated). It complicates and delays the re-patriation of the minor |

Yes your 4 month old baby does need a passport to travel with you to Spain. It will be a passport that is only valid for 5 years and costs nearly as much as a 10 year passport. |

Yep. A ten minute old baby would need a passport.
Also, if you are not traveling with both parents - you need a notarized form from the other parent stating it is OK for you to leave with the child and when you will return. |
