Another vote for Holland. When I visited there, you didn't even have to ask anyone first if they spoke English, everyone speaks English - young & old.

Most people in Italy speak English. In France, I sensed that most people spoke English, but they respect you more if you attempt to speak French first. In Germany - all young people speak English, not so much for older people.

All of the Scandinavian countries speak English (Norway, Sweden, Denmark) and also Finland and Iceland.

One place I felt they weren't very friendly towards Americans was in Austria.

Come to Greece. Most people here speak English and those they don't they speak the body and hands language very good to understand.
Nice people, nice beaches and much of history and archeology not to miss cruising the Aegean Sea

In general, much of Europe is multi-lingual with people who speak some English working in most hotels, museums, and cafes. Some people have had a good experiences in Holland and Belgium. Each time I have travelled to different parts of Europe I have done well with limited language skills.

Though it is a great way to show respect to learn at least a little. Even if it is just to say thank you, how much does this cost, and what is this (on the dinner menu).

Copenhagen in Denmark. You don't really have to even ask if they speak English, even people in the 7-11s did. Also, Frankfurt in Germany. It is a very international city and most people speak English.

Amsterdam is best, virtually everyone speaks English there.

Any city in Germany. A large amount of Germans speak English, and with a German-English dictionary you shouldn't have any problems.

Italy! Siena, Italy to be specific. You would be surprised at the number of people who speak English in Italy. Because Italy is visited by so many tourists who speak English, it's hard to find a place where no one speaks English.
