This is a bit of a minefield as some of the ones mentioned are not just straight switches in name only (they are evolved from changes within the territories)

Here are a couple which are safer to use with regard to a name change:
Indonesia was Netherlands East Indies until 40's
Guyana was British Guiana until 60's
Belize used to be called British Honduras until 70's
Sri Lanka was Ceylon until 70's
Burkina Faso was Upper Volta until 80's

Kampuchea is now Cambodia, unless they changed it again.

The Czech Republic and Slovakia used to be Czechoslovakia
Zimbabwe was formerly Rhodesia and before that was Southern Rhodesia
Zambia used to be Northern Rhodesia
I forget what Malawi was called previously
Prussia has disappeared from the maps
and where was White Russia?
Germany is a relatively recent invention - it was fragmented into lots of principalities
Poland has had a chequered history - the territory we now call Poland has had lots of names in the past
Belgium used to be part of The Netherlands
What used to be Yugoslavia was formerly several different states, and now it's fragmented again ... can't remember all the names of the new countries
Botswana used to be Bechuanaland
