Mr Who
There isn't a (free) "Fahrschul-Fragebogen" (a document that you could download) to be found on the Web; neither in German nor in English. Sorry for that. You'll find my findings confirmed if you do your own search with the the words that I put in quotation marks.

There are, however, sites where you can brush up your knowledge online - such as http://www.fahrschule.de/Testbogen/

I took my test in English, the theory was in English, and I had English study material, it did cost me 60 euros for the book from a local book store, and I got lucky with an English speaking teacher.

There are some on the net if you search for "Fahrschulprüfung Theorie", but be careful not to fall into some trap, as if you click "ok", you agree to pay some dozen euros.

A German driving license is not cheap; it will cost you some 1,500 euros, all lessons included, and if you don't speak German by the time your "theoretische Prüfung" comes around, here's the good news: They DO HAVE questionnaires in English. But by the time you'll take the test, you won't need them anymore, because you'll be so familiar with the German questions that any other language would pose a problem to you.

And learn to drive a gearshift car, for whatever means. NO, and I mean NO, German drives an automatic, except the physically handicapped and old housewives.

There is no free website, but is possible to buy the questions.

Germans usually have to pay for the Fahrschule-Fragebogen, so it makes no sense to make them available online for free in English.

A good source of getting all the materials you need for German driving school is www.toytowngermany.com Just post a request in the forum, there is bound to be somebody there who has just completed the test and is willing to sell hir stuff.

Another good source can be the websites of the US Armed Forces in Germany, but their information usually relates to the special driving permit members of the US Armed Forces are allowed to hold in Germany, so take everything with a grain of salt.
