You might go to www.telefonbuch.de and give in name and address. Then you might have the chance to find her. But just in case her husband (if she is married) is listed in there you won't find her cause mostly just the husbands are listed. If you know her husband's name then it might be easier.
I tried to find her with the informations you gave us here and didn't find her....

Here is a German site but in the english version for you in case you do not speak German.
Often with woman it can be difficult for last names change often if they get married. Do you know any of her friends or family that may be able to find her for you? I have done it that way before when I lost contact with a friend there, I had one that knew them contact them for me and we reunited.

You should contact the town hall in the city where your friend used to live. In Germany people need to register their residency at the local administration. You want a "Melderegisterauskunft". Fee for this is usually 5 EUR, payable by cheque.
