You tip the waiter or waitress in cash if you pay cash.
There's no rule for tips. It's usually linked to the service.
Good service good tipping. Bad service bad tipping.
Round up to a sane amount in the range of 8-10%.

Tipping in Germany is definitely NOT like it is in the USA. There is no 15% rule or anything like that.
In fact, the service is included in the final price of your meal. ie.. the tip is included in the price of the food.
If your bill comes to something like 28.50 (purely an example) and you give 30.00 even, that will be fine and nobody will be insulted. If you give too much, however, you may actually be asked why by your server. The mentality in Germany (and many places in Europe) is quite different than that of the US and Canada.

It is already included. But if your waiter/waitress is good tip more. I've tipped extra even though I knew it was already included. If the service is great, show it.

The tip is always included in the prices of German resturants/bars etc. If you want, you can leave some tip on the table, kind of rounding up.

Service and all taxes are included in the bill in Germany.
But if the waiter/waitress was ok you can give tip.
Per example: Amount 18.50 € - I would pay 20 €€.
