You didnot mention whether you want Indian passport or some other.

For Indian -

You can apply for the passport right from day 1. All you need is a birth certificate and passport photos.

If the child is not born in India, but has Indian parents, then you will need to provide copies of parents passports as well.

For US passport -

again you can apply from day 1 - if you go to your local postoffice with the birth certificate, they will give you forms to fill up and give you all other details. Some post office also take photographs there, so minimize your running around.

You can apply for the passport any time.Contact an agent he will tell you the papers needed a rough list
1. 4 passport size photos
2. Copies of the passport of both parents.
3. copies of proof of residence of parent
4. Copy of the birth certificate
You might have to pay the agent about 2000 rs for processing.That's a better idea to avoid the rush and hustle at the passport office. Since its a baby I don't think any other formality is required

which will be issued for 5yrs only.

or they will add the minors name in the parents passport.

You can get an American passport for a child of any age. Check with your local courthouse. You will need a birth certificate. Make sure you do this well in advance of your travel because it takes time to process. Have a great trip!

She can have her own or you can have her recorded on your passport. Either way. However since you have family abroad and so are likely to take more trips there in the future you'd better apply for her to get her own. Contact the City Hall of the city where she was born.
