It is very likely you will not get US Visa from the Consulate and your dreams for going to US appears to be very bleak. If you are a trained and qualified Nurse you can possibly get a visa as they overlook many things only in this category.

Notary certification is done for Rs.25/- and US consulate people know all these. They are also not accepting sworn affidavits in court, now a days. Any way best of luck-

You don't need a birth certificate any have. Bcos your passport is the latest document. But, any have, you try to compensate the mismatch, which may fetch you some serious troubles in future verifications.*

First of all I do not think that US Consulate verify the SSLC Certificate, Driving Licence or Birth Certificate during VISA processing. Hence I do not think that it will cause any problem.

However,if you have time with you, it will be better to get the date of birth changed in the Passport with the help of birth certificate.

get date of birth in passport corrected from passport office before u go to us embasssy, other wise ur visa application may be rejected

If you are looking student visa, or somewhere embassy has asked for dob certificate only then problems may I repeat MAY arise otherwise dont bother.
u can consult with an IATA agent from whom you will buy tickets

Dear,Just take your passport,Bank balance statement,business document,I.T returns with PAN card
Keep your Driving licence in your pocket & B/Certificats at home.All the best

You don't need your birth certificate if you already have a passport; that's all you need for travel - that will also act as your ID

Once you have your Passport, you don't need your Birth Certificate, embassey will also not ask for the Birth Certificate.

Forget that you have a Birth Certificate.
