Failure of security agencies cumulatively. What were the intelligencie agencies doing? Where was the Navy when coast guard boat was hijaked? This was a well organised, orchestrated attack consisting of Alquaida, Palestenian Guerillas, with Pakistani mercenaries. It is to be noted that main target was Jews(mostly from Israel). There was a Jewish congregation arranged in that area. About 30 persons hijacked a Coast Guard boat and this was neither intercepted not any steps were taken to alert. Adequate security measures were not taken for the Jewish congregation. Jewish community is the target for Palestenians and Neo-Nazis.

A gunman, holed up in Mumbai's Oberoi Trident hotel where some 40 people had been taken hostage, told an Indian news channel that the attacks were revenge for the persecution of Muslims in India. "We love this as our country but when our mothers and sisters were being killed, where was everybody?" he asked via telephone. No answer came. But then he probably wasn't expecting one.

I always believe that --
"Chance favours the prepared mind" (Chance favours only those who are well prep aired to face the situation).

And I think Terrorist are well prepared, then our security personnel.

If our Security personnel are well prepared, then it might be a different story like.
"Mumbai Police arrested 25 Terrorist, and seized large amount of Hand Grenades & AK 47.

Hope I future our security personnel will be well prepared.
And our Police Department, Army, NCG and every single person directly or indirectly help to complete the operation..

they were terrorist attacks. in a city of millions, it is hard for the gov't to keep an eye on everyone. yeah the gov't should have been prepared for something like this at any time and they could have done more, but we can't blame the gov't for everything. if this was destined to happen it was going to happen.
