I've lived in northern Italy for almost ten years and go out exploring on the weekends. The weather can be variable depending on exactly where you are, but it's generally a good time to be here. Most things will be open, but without the huge masses of tourists. Here in northern Italy, we can get fog starting around the 2nd or third week in October, but that isn't a big problem if you're not driving. It can make places like Venice pretty magical - especially in the early morning when there aren't many people out and about.

It can be wet especially toward the end of the month in some places. It's useful to carry a small umbrella just in case.

There are some interesting festivals in October - one excellent one is the Eurochocolate Festival in Perugia. The Palio del Papero near San Miniato in Tuscany is interesting. It features a costumed parade and competitions between the four neighborhoods of the town that take the form of duck races. There's also a lot of good food. The Bardolino Wine Festival on Lake Garda and the Rice Festival in Isola della Scala just south of Verona are pretty good too.

I was in Tuscany the first week of October and it was wonderful (70's and sunny). Plus, the crowds have thinned a bit and there are low-season prices. You can review the average temperatures for any place (which is important since Sicily isn't the same as Milan) by going to http://www.weather.com and searching for your specific destination e.g. Florence, Italy, then click on 'month', scroll to the bottom, and click on 'averages'.

i'm italian so...i think i can answer to this question!
it isn't too cool but it isn't hot neither.
sometimes rains but NEVER snows in october (usually snows in november, december, jenuary and february)
north: rainy and a little cool
center: not much rainy and not much cool neither
south: it's still summer
enjoy italy, weather is always good here
