Literally it is translated to "Sempre e per sempre" but that might sound a bit awkward to a native Italian speaker.

The most natural translation that an Italian would use, in my opinion, is simply "per sempre", which translates literally to "for always".

Anyway, the litteral translation is "Sempre e per sempre" but it sounds better to say "Da sempre e per sempre"

yes it can be trastated with "sempre e per sempre" but doesn't exist a form used in spoken Italian....
a good traslation could be "ora e per sempre" "now and forever"
