The easiest way to get there is to rent a car - it's 170 km (a little over 100 miles). However, you can also use public transportation.
From Malpensa, there's a bus that goes to the main train station (Milano Centrale). The bus runs twice an hour, costs 5.50 euro and nominally takes 50 minutes (traffic conditions can extend the time - in my experience, it's often an hour). You can buy tickets from the driver. The bus is at stop 3 in front of exit 6 from the airport. http://www.sea-aeroportimilano.it/en/malpensa/index.phtml?mod=per_mi_scroll&to=come_arrivare. You don't want to take the train from the airport because you'll have an extra leg for the trip on the metro to get from the Cadorna station to Milano Centrale Station.
At Milano Centrale, take a train to Desenzano on Lake Garda - it's the station for Sirmione. There are 1 or 2 trains an hour; the trip takes 1:07 hr:min or 1:23 hr:min depending on the type of trains. The faster intercity trains cost 13 euro (2nd class) and the slower regional trains cost 6.75 euro. You can check specific times and prices for trains at: http://www.trenitalia.com/en/index.html. Note that the train will be going on to Verona or Venice - the displays in the station will indicate the endpoint for the train and not Desenzano.
From the Desenzano station you can catch a bus, taxi, or ferry to Sirmione. The bus line for that part of the lake is seasonal and doesn't run through the winter, but if you're there in Spring/Summer you can take the bus going to Verona.
The boats don't run between the 4th of November and the 8th of March. If you're there when the boats are running, the schedule will be at: http://www.navigazionelaghi.it/ita/gar/nlg_ora.html. It's about 20 minutes from Desenzano to Sirmione on the boat and there are good views of the castles of the two towns from the ferry. It's a short cab ride from the train station to the port in Desenzano.
There is a second train stop on Lake Garda at Peschiera del Garda. It's a little further from Sirmione than Desenzano. |

Take the Malpensa Express train to Milan Cordona. This is run by the Ferrovie Nord railway and you'll need to get a taxi (or metro if you can get your luggage down the stairs) to Milano Centrale station for the FS train. From there, get a train to Peschiera del Garda and a taxi from there to Sirmione. |
