It depends upon 3 things:
1.) Where in Italy you're dining
2.) What type of venue you plan to eat in, and
3.) What season/time of day it is

Italy in general is like anywhere else; there are rules to fit any accomodation. You wouldn't dine at the Four Seasons in shorts and a tee shirt, nor stay at a Youth Hostel and wear silk pajamas to bed! Milan, Rome and Venice can be fairly formal when it comes to evening attire, but there are still dives that don't expect tourists to be decked out for a meal

High heels and generally a dress or a very dressy top.

It depends where you are eating! If it's a "trattoria" you can wear whatever you want but if it's a chic restaurant that might be embarassing.

Flip flops are for beach wear, not city wear. If you need to wear open toe foot wear then get some open back sandals.
Blue jeans that complement you shape would be find. Remember to accessorize, bracelets, earrings, scarf, belt, necklace... and a bit of makeup.

it depends on where you're eating. dark blue jeans are better than the lighter ones because they look dressier. flip flops are usually a no if you're going to a nicer restaurant, but if you're wearing them with a summery dress they will look better.

It depends what kind of restaurant it is... If it's a really fancy restaurant I would not suggest them.

well I think you are not going to a wedding, so i don't think is a good idea use an high heels or some dress or a very dressy top. or are you going to ... A flip flop sandals to dinner can be great, ...
better be careful with your stuff , and attention to the prices, they will try to " fregarti " (stitch up)
I was there 2 times (all of them bad experiences)
