For weekly rentals in Italy, I have used www.1800flyeurope.com many times. They have always worked out the cheapest for me. You'll pay in U.S. dollars and you'll pick up your car at a Hertz counter.

If you change your mind and decide to only rent for a few days, I'd recommend an Italian company - Easy Car SpA. - www.easycaritalia.it. The site is in Italian, but it is easy to figure out.

Public transport system is excellent and reliable, not sure about renting cars!
