If you want to visit the Vatican museums and the Sistine Chapel you can book in advance through Octopus Travel or ViaTour. We booked through Octopus and it was brilliant. Have a look at their website.

Hi,,, you can go stand in line like everyone else until you get up there,, and purchase them for the next day...

Now, for the Cystine chapel,, you better go EARLY,,, in the morning,, and find out what days they are open... not open every day,, and close at Noon!!!!!

Beaware of the pickpockets,, they are very good...

You can fax a specific number at the Vatican which your hotel might be able to help you with. However, you won't know whether you have gotten in until the night before! Alternatively you can book a private tour - however that can be expensive as they only do a minimum of 3 hours. Otherwise it is joining the queue at some hideously early time in the morning. Hope that helps. The Sistine Chapel is amazing, don't just look at the ceilings the frescoes by Perugino and others are amazing too - however don't lie on the floor to focus on the roof as you will probably be arrested! You can't miss Raphael's Stanze before you reach the Sistine Chapel. Have a marvellous time.
