The cruise line for sure organizes tours - it may cost 200 bucks but I'm sure a meal and guide (and of course ground transportation) is included. Go with one of the tours offed (for Naples) and you'll avoid getting ripped off in a city full of people looking for an easy target.
Get me right Naples and Neapolitans are great folk (bright & witty). But, right off the cruise ship and looking around for souviners in Naples makes anyone an easy target for the husslers.

If you have not visited Pompeii it is certainly worth a visit - fascinating and they do walking tours which cater for the amount of time that you have i.e. you follow different coloured lines on the map depending on how long you have available

Enjoy your cruise and your birthday!

Sure on borad they have got some opportunities but if you write me what are your preferences I can help.
If you have just few hours I think it is hard to go to Pompei Capri or Vesuvio. But may be you don't care to hurry and have a rush. Let me know
