Hi,
First buy yourself a book on the different regions of italy and see which suits your vacation desires the best. North, Centre or the South and islands all offer something magical it is a matter of personal taste. Most people make the mistake of squeezing in the major cities, Venezia, Milano, Firenze, Roma, Napoli/Pompei/Amalfi to see what they think are the "must -sees" and miss out completely on the meaning and experience of an italian holiday.
There is entirely too much to see and do in every region, cuisines vary by regions, you could stumble upon on a small town in tuscany and find a church that defies comprehension.
Roma and milano are great for shopping, as for history and culture they are represented everywhere in the country. Every city and town no matter the size has piazza's where you can sit and have a coffee. Do you want to go strictly to cities, or do you want to go to the sea or countrysides? How much time do you have for this trip?
One popular solution for first timers is Firenze and Roma - you can plan other side trips in between and still have had the opportunity to see plenty of "must-sees" ie, David, Botticelli's the birth of venus, Pisa, Montepulciano, Lucca, Viareggio, Caravaggio's works, the Pantheon, the Vatican, The Colosseum, Piazza Navona, Castle St. Angelo, Fountain of Trevi, the catacombs, Hadrian's Villa, Tivoli, the forum, the ruins, the mouth of truth, the major cathedrals of roma and so much more.
