In Italy, how should I tip, the same as in the USA?

in Italy it is not expected to tip. you will not be looked at as cheap, they automatically add a charge to your meal to cover what the tip should be. so if you did tip you would end up paying upwards of 40 percent or more on top of the price of the meal.

also in Italy they don't usually serve drinks w/ice cubes, you can ask for them and they might be able to get them for you.

I have found that in many European cities it is not proper to tip. Once, while in Spain' I was told that tipping is almost an insult and that the waiters/waitresses get paid and that is all the money they get. Tipping is much more of an American thing I guess. I could be wrong but this is what I have found through my travels

it depends on how much you eat

Often Italians tip the quality, goodness, and satisfaction for the food they eat, rather than service that could change of course by tourist time crowd.

In Italy it's not an obligation to tip.
I usually tip when I'm really satisfied with the service.

Ok in Italy you pay a cover charge in restaurants. So depending on where you are, it could range from 1.50 euro- upwards per person. I usually only tip the actual waiter or waitress if they have been extremely friendly and professional. I find that in a lot of restaurants the service can sometimes be quite sloppy and unprofessional. They do expect a tip especially in touristy places, but like i said leave a tip only when you feel its necessary.
