Definitely depends upon where you are going to be spending your time. We rented a car relatively reasonably through AutoEurope.com and had no problems at all....except driving in Rome is without a doubt worse than driving in ANY US city I've ever driven in or through. If you're going to be spending time moving from city to city (Venice, Florence, Rome, Milan) and not spending much time heading to the outskirts of town you can get around quite easily by train.

we just came back and had a rental car the whole time. it was nice since we stayed in a villa out in the country and drove into the cities and around the countryside. parking was expensive, as was gas, but it was worth it being able to move around on our own schedule and not rely on the trains and such. we did park and use the subway in rome, so much easier. my husband enjoyed driving on the autostrada. driving is confusing as hell since there are tons of signs and they all block the one you are looking for. highway driving is easy and little hassle. its the city driving and finding good parking that can be annoying. be prepared to have crazy drivers and streets with no existing lane markings. also, get the smallest car possible that will still work with your load needs. our little smart forfour was perfect and really managable in some tights spots.

Depends where you are going in Italy... If in the country, probably yes... If the city then no.

In Rome you can take the bus or subway almost eveywhere so it wouldnt be necessiary. out in the country a car would be nice but trains can take you many places. Gas is also expencieve and some European drivers are crazy.

Depends on your trip plans. In Rome a car would be a burden. Parking is very expensive, and driving is difficult. It is better to use public transportation.

The same is true about Milan. In Venice or Cinque Terre it is impossible to use your car even if you have one. However, if you don't plan to stay inside a city, having a car will free you from the schedules of busses and trains. Driving out of town is not too difficult.
