In Italy there are 2 Montepagano.
The closest to Naples is in Abruzzo (province Pescara)

It might depend on how you want to travel through the city. Traffic is rather choatic. If you go by train you should be prepared to pay some discheviled resident who will undoubtably approach you 1-2 euros for the purpose of escorting you to your train unmolested. Naples is a beautiful city if you can stay in the right areas, I'm sure its underrated and probably skipped on a lot of itineraries because of what guide books say about it.
