It really depends on what you're interested in and when you're here. There isn't any absolute best that fits everybody. Certainly, if yo've never been to Italy, the major destinations - Rome, Florence, Venice - offer a lot to see and do with plenty of opportunities to get out for day trips. If you're interested in nature and scenery, then the lakes area (Garda, Como, Maggiore, ...) is excellent. I live about an hour from Garda Lake and spend a lot of time there throughout the year. Other possibilities are Cinque Terre and the Amalfi Coast.
A lot of the smaller cities and towns are well worth seeing and there are many interesting festivals throughout the year. Next year, the Biennale will be running in Venice in addition to short term events like Carnevale and Il Redentore. The Palio in Siena is a major attraction, but other versions are also great and less crowded - like the Palio in Ferrara or the Giostra in Arezzo.
If you're interested in castles, you can't go wrong just about anywhere in Italy. There are some particularly nice ones in the area around Parma.
Perugia has the Umbria Jazz Fest and Eurochocolate festivals. Both are world-class events in a city that's interesting even with nothing special going on. The 14th century buildings which are now underground are used as a part of the venue for the chocolate festival.
There are a number of beaches which are worth visiting if you're here in the summer.
It would be hard to go wrong no matter what you choose. |

We visited Italy in September, 2007. We flew into Rome and stayed 4 days then traveled by train to Florence where we stayed 3 days then we took a train to Venice for 3 days and back to Rome for a day before coming home. While in Rome we visited Pompeii for a day.
We thoroughly enjoyed Rome and Venice. I liked Rome, my wife liked Venice best. It is a toss up. Florence was good too, especially the museums. I was overwhelmed by Pompeii. I found the quality of life in Pompeii at the time of Vesuvius, 67 AD, to be amazing.
When we go back we will visit Rome and Venice again and also Tuscany.

If you have time spend a few days on the Amalfi Coast, and a couple of days walking at Cinque Terre. You could also consider Bari and Brindisi, beautiful cities that not many tourists visit because they are so far south.
