What are the general living standards of Italy?

Italy have the seventh largest economy in the world, so it's a rich Country.
It's absolutely true there are disparities and different social classes, but like everywhere worldwide, including the US or Japan.
So, if you mean "compared to the rest of the world", people in Italy are mostly rich.

mainly middle class, but you will find there are more rich people in the northern regions, and poorer people in the southern regions.

Most of Western Europe has the same "average" standard of living, whereas the Eastern part is strivng to catch up after 50 years of "planned Economies", with the same differences between the poorer parts of the country and the richer parts. You will find "richer" Milan and Munich at the same level and the "poorer" Palermo and Dresden with the same problems. There are also periods of higher and lower general affluence; at the moment a wave of problems is sweeping the middle classes all over the Continent and Italians are not immune. The main themes in Italy are National debt and Tax evasion, with the Government making every body's life miserable in it's attempt to close the gap. Another theme is Juvenile unemployment, with the South peaking the polls and a crisis in the Fashion Industry's export drive (China effect), with the North suffering most.
So what are the Italian standards of living ? Average West European. How do Italians live in general ? Apart from pockets of penury, quite well thank you. The weather is great, the food is even better, you can enjoy shows and the theatre and even stroll the streets and just meet people and see the sights. Count the cars and phones per person, that's a clue.

ps as far as "eating off" I'd say "eating in the streets", in some of the most colourful cafÃ¨s and restaurant of the World. It's an old Latin and Greek custom, goes back to the Roman "insulae" and is favoured by the mild weather. Young Julius is said to have pratically lived in the shops and snack bars of his Aunt's insula in Republican Rome. Makes one think where "our Civilization comes from.
pps in terms of cleanliness, remember who "invented" the baths (the town of Bath in Britain still has the remains of well furnished Roman baths) and Today in no self respecting Italian bathroom is the "bideÃ¨" missing (note the well known joke about "washing babies in").

In Italy most people are in the middle class but the middle class in the last year is shifting down.

I have been to Italy so i would say mostly middle, its kind of similar to Greece, although it depends from region to region.
