This is a hard question and would cause many an argument among the Milanese who still live by the century old ideas of campanilismo (sorry about the spelling).
The town centrists would claim the il Duomo is the most famous church in Milan. It is the third (maybe fourth) largest in the world, and is about 700 years old. It is the base for the dioces of Milan. However, the Duomo has been under reconstruction for years and is now masked in drapes.
On the other hand, some northern Milanese, would claim Santa Maria Della Grazia is the most famous. It houses il cenacolo (the Last Supper, by Leonardo.) Il Cenacolo is by far the most important piece of art in Milan.
Lastly, the truly devout would claim Sant Ambrosiano, the burial place of Milan's patron Saint is the most important.
Hope this helps. Personally, I think the most important churches in Milan are located on Via Montenapoleone. The churches of Gucci, Prada, and Versace. |

I think mtpizzo is right if we talk about the level of history.
But generally, for people who are not Milanese, and specially not Italians, Duomo is the most well-known one, since it's used like symbol of Milan.
And of course, the one at Via Montenapoleone is adorable ! |
