I live about a half hour from Modena and spend quite a bit of time around Lago Garda. Take the train from Modena to Verona and change there to the train to Peschiera del Garda or Desenzano. Both are towns on the lake and there are boats or buses to get around the lake.
You can get to Lago Como by taking the train from Modena to Milan (Milano Centrale station) and changing to a train from there to the Como San Giovanni station.

The rail network in Italy is brilliant - reasonably cheap, invariably punctual and decently comfortable. I have certainly been to Como by train, so I'd look into that as an option first.....

if wish to visit portofino 2 then get from modena to LA spezia by train or rented car, see portofino and then from there get to Milan from milan get train to como! easy done, luckly for u in europe distance are very short!

ps but if r going to portofino then i d advice to cut off lake of como and
get to 5 terre (fivelands)which are amazing and close to porto fino.
