"Tuscany" and "Toscana"?

I'm very confused with these two words! Could someone please tell me how different? Tuscany is an adjective form of Toscana? Or both are noun? Or one is English and the other Italian maybe?

toscana is often used as an adjective to indicate that something is from the tuscan region. And plenty of Italians say "Tuscany"...
