for most of the year, continental's flights out of houston are cheaper than other carriers out of miami. texas and florida are definitely the two cheapest (and fastest) in terms of flights to central and south america.

Florida. They fly over the Caribbean. Next cheapest would be Texas. Fees from both FL and TX are about the same. It all depends on when you fly.
