Use Mapquest, it works.

You just follow the highway signs to San Jose from Manuel Antonio (not too hard, there really aren't any real options). After you pas the international airport in Alajuela and the toll gates you need to go to your left (East).

There are some good maps of Costa Rica on this site. http://www.govisitcostarica.com/travelInfo/map.asp

Return towards San Jose upon reaching the end fence of the airport that will be on your right, turn left as if going to DENNY'S then turn right again into a small town called Rio Segundo, just follow this road for about 20 min and will reach downtown Heredia.

Or Or
Return towards San Jose pass the airport and continue on the main road continuing towards San Jose as you near the Cariari Mall on the left, exit on the right to circle around and over the pista keeping the cariari mall on your left and follow this road into Heredia

The first choice is really the best and fastest

Hope you enjoyed C.R.
