Unless your health issues prevent you from having lots of fun too, why stay in one place? Turn it into a health vacation. Spend a month on the west coast, and month on the east coast and a month somewhere in between. There are so many great places in CR having 3 months to really enjoy it is a great opportunity.
I've spent time in Manual Antonio, Monteverde, Arenal & San Jose. I'd recommend all of them accept San Jose (big city with big city crime). Arenal is a volcano though, so if your health issues are respiratory related, that one may not be a good one - but the hot springs can be very healing.
There are a lot more places to see. I've just started my exploring. Next year I'm probably going to do Montezuma or Tortuguaro. |

The safest place and most beautiful is in the northern pacific coast. You should go to Monte Zume, im sure you will love it and its close to playa tambo, another beautiful place. When its winter in the states it is summer time here in CR so you will have no problems with cold weather.
Below is a link of the Nicoya Peninsula where Monte Zuma is located.
Im sure you will also make a lot of cool friends there also.

WE send many single women to learn Spanish in Costa Rica at http://www.spanishabroad.com/studyabroad.htm and I haven't had a safety incident yet. Costa Rica is fairly safe though crime exists. I would think you would be safer there than most cities in the US. I live in Phoenix and certainly felt safer outside this city. Of course you don't want to let your guard down as we truly are not safe anywhere in this world.
San Jose is fairly safe if you choose the right neighborhood, but you may feel more comfortable in a smaller town like Manuel Antonio, Tamarindo, Fortuna (Arenal), or Samara. Heredia is a good option for the San Jose area if you want a more quiet suburban environment.
Though I don't offer programs on the Caribbean coast and my views may be slanted by headlines, I hear most crime and bad incidents come from this part of the country. Poverty would be the clear indicator, but I am sure you could spend months in Limon or Puerto Viejo with no problems, however I would avoid this part of the country based on my bias. Some my say downtown San Jose is unsafe, but you really have to weight the advantages of a large city to its possible disadvantages. The city certainly has less tourists than a lot of beach locations. |

its a reasonable price for all people
the corcovado park is close, the air is clean and the place is great
i went there a couple of months ago and it was amazing
in the cottage we stay, was funny cuz when it rained we had all this cups around do to the holes on the roof, everyone enjoyed
the sounds of the rain and droplets, with the breeze... i know you won't forget cuz i haven't
the temp there is great

I would recommend a warm place that has a medical center near, or at least has quick access.
If you are as concerned as you are for your health, it would be my best bet, but you left very little information. You don't tell how much you can afford or what kind of warm place, since Liberia its hot as hell but its a small town, but so is Manuel Antonio and is a beach.

I agree with Kristina's answer, move around. CR has so much to offer, in reality 3 months is just the appetizer and you do not want to spend 3 months in one location. If you have health problem, the hot springs are great. I will add, try and keep away from the tourist trap cities (San Jose, Jaco, Limon etc), stay in hostels where it is cheap and meet people where you can share experiences.

That depends on what you like, Atenas, Grecia, Naranjo, Palmares, Sarchi, Heredia, San Antonio of Belen, La Sabana, San Pedro, Santa Ana, Villa Colon, Palmares, Escazu are all favorite places.
the ones outside of San José tend to be cheaper.
Most all places are safe, just use common sense and you´ll be fine.
