Have your friend call his cell phone provided and request World wide calling. It's only about $6 per month. Raoming charges are about $2 per minute. For local calls down there purchase a local phone and minutes.

I go to Ecuador all the time. Tell him to buy a Porta card or BellSouth card. They are inexpensive and you can use them on the little public telephone cabins that they have.
OR
He can go to one of those international calling places, they have little cabins that you can call the US for about 15 cents a minute.
Both are easy options :)
