Montevideo, Uruguay
Quito, Ecuador
Mexico City or Guadalajara, Mexico

Antigua, Guatemala....I've been checking it out (been there this year) and intend to return in November for some courses. There are a lot of cool places to go, but one must really study the do's and don'ts of Latin America Travel.

Antigua, Guatemala
I know a bunch of people who have studied spanish in Antigua and have said that it was very good. There are many different language schools available and the city is beautiful and is used to many Americans visiting. Plus most of the language instruction is 1-on-1 with your teacher, so you can advance much faster than if you were in a large classroom of students.

I am living and teaching in Honduras right now and although it isn't like the states, Central America is a great place. Like anywhere outside the US, use some caution when walking around and be smart. As long as you use your head, you will probably be fine.

Just don't go out wearing your shiny watch and your name brand cloths. And don't wonder around places you don't know. I'm from Peru, I don't know hoe other countries are but that's all the advise I can give you.

Costa Rica is safe and their Spanish is one of the best in Latin America.
